Health Office Forms

Allergy Action Plan 

  • this is required for any student who has allergies and may need an antihistamine or epinephrine  (epi-pen) for an allergic/anaphylactic reaction. 

Asthma Action Plan

  • this is required for any student that needs to use an inhaler while at school. 

Authorization for Medication at School

  • this is required for any student that needs to have ANY medication while at school.  

Seizure Action Plan

  • this is required for any student that has a seizure disorder.   

Student Physicals

Private Physician Physcial (NOT FOR SPORTS)

  • this is required for any student who is NEW to the district.  THIS CAN NOT BE USED FOR SPORTS.

Sports Physical

  • this is required for any student who is wanting to play sports.  The ELIGIBILITY page is turned into the nurse when private physician is completing the physical.  All pages are needed if school physician is completing the physical.
